The questionnaire is designed to measure the impact of oral health on the quality of life of children aged 8 to 15 years. It consists of 34 questions and asks the child to rate their own oral health and how it affects their daily activities, social interactions, emotional well-being, and overall quality of life. The 34 items are scored on a scale of 0 to 4.
